As Riverside’s Care Relationship Manager and Community Liaison for the state of Wyoming, Heather Cook, RN, is dedicated to building meaningful relationships with clients, families, referral partners, and care teams. Her role centers on connection, advocacy, and ensuring a smooth transition home for those in need of compassionate, high-quality care. Each day looks different, but at the heart of her work is helping families better understand their options, coordinating thoughtful care plans, and walking alongside them during vulnerable seasons.

With 17 years of experience as a Registered Nurse, Heather brings both clinical expertise and heartfelt compassion to every interaction. Her background allows her to bridge emotional support with informed guidance, helping families make confident, well-informed decisions rather than reactive ones. She understands firsthand how overwhelming healthcare can feel, and she is passionate about bringing clarity and reassurance during life’s most challenging moments.
